ATTENTION PARENTS! Students needing any medication, including over-the-counter, to be given during school hours MUST have a medication authorization form on file in the school office. Please go to www.ilcsw.org, select "School - home" and click on the "Medication Authorization Form" to print out a copy. A physician's signature is required for all prescription medication.
Please remember:
1) A new form is required each school year and your initials on the registration form does NOT take the place of this Medication Authorization form. However, if you have already obtained a Physician's order for this school year, that is acceptable.
2) Parents must supply the medication in a container with the original label. Medication NOT in the original container will NOT be accepted.
3) If your child has a history of ASTHMA and ever uses an inhaler, even if it is rarely, please supply the school with an inhaler and the appropriate forms, including an Asthma Action Plan completed by your physician. Stop by the school office if you need this form.

1) If your child is complaining of feeling ill check his/her
temperature before sending him/her to school. Students with
a temperature of 100.0 F or greater should NOT attend
school. Students need to be free of vomiting, diarrhea,
persistent cough and fever free without Tylenol or Ibuprofen
for 24hrs before returning school.
2) If your child is diagnosed with a contagious illness, such as Strep,
Whooping Cough, etc., please notify the school office as soon as possible.
Also, please call with any new health information about your child (change
in medication or dose, new diagnosis). The more informed we are the
better we can care for your child while he/she is here at school.
3) Remember...good nutrition, practicing thorough, frequent hand
washing, along with adequate rest, is the most effective way to prevent
illness. In addition, remind your child not to eat or drink after anyone!
